當患者卡在樓梯這一關——不敢落梯、覺得手術腳撐不住、或者不確定角度是否足夠轉為交替——就是尋求專業評估的時機。上門評估時,治療師會用家中真實的那道樓梯教授步序,而非診所的標準樓梯,因為唐樓、公屋每道樓梯的級高、深度、扶手位置都不同,在真實環境練習才能安全過渡到自行上落;亦會先量度膝屈曲角度與大腿力量,決定當日應該用一步一級,還是可以開始嘗試交替、是否需要拐杖或手杖,避免「日數到了就強行轉換」而跌倒。這種由 step-to 過渡到交替上落應由角度、肌力與信心決定,而非只看術後第幾週的判斷,屬臨床共識,會逐次覆檢。Nova Health 的註冊物理治療師提供的上門物理治療,會按當時的膝關節狀況與家中實際樓梯狀況,調校節奏與輔助;官方指引提供的是步序原則(好腳上、壞腳落、用扶手),實際執行的細節則靠逐次評估收窄。
